Perhaps the design of light rail in Houston is flawed or perhaps it's simply the inability for people to drive properly, regardless this video is a compilation showing that something is obviously very wrong:

man, that's just bad...  seems most of the were folk turning left in with the train coming up from behind...

Yeah, I'm not sure the entire blame should go to the driver (though they are obviously at fault in these cases). When you are making a left on roads without a streetcar/light rail you only have to worry about folks coming toward you so it gets ingrained into your driving habits. I bet none of these drivers will let it happen again though.
